How to Bill and Code an ultrasound injection or Arthrocentesis of the Ankle

There are many reasons to perform an ultrasound guided injection of the ankle as a Podiatrist.  Also, there are many reasons to use an ultrasound machine to perform said procedure.   Now the first key to billing the injection of the ankle is billing the correct CPT code for the ultrasound guidance for the procedure. The code used for that is 76942, which is the ultrasound guidance  for needle placement.

The CPT code for the arthrocentesis is 20606, which is defined as arthrocentesis, aspiration or injection of intermediate joints or bursa with ultrasound guidance and performance.

If you own the equipment you are allowed to bill the global fee without any modifiers.

Now to be clear, if you have decided to bill this it is extremely important that your documentation of the ultrasound examination is thorough. In other words don’t perform the exam on some cheap 1950’s ultrasound machine.  Ultrasound machines today are inexpensive and available on Amazon here is a simple one you can use:
